Bleed, the Ravenous Blade

Weapon (long sword), legendary (requires attunement)

A long slightly curved blade that is always dripping with blood, Bleed is a terrible weapon that consumes the life force of all those around it, including its wielder. This sword comes with a silver sheathe inscribed with ancient runes capable of dampening its magic.

You gain a +3 bonus to attack and damage rolls you make with Bleed. Addtionally, this sword deals an additional 2d8 necrotic damage to any targets it hits, and its wielder regains an amount of hitpoints equal to the necrotic damage dealt this way.

When this sword is drawn from its sheath, each creature that doesn't have full cover within 30 feet of it, including the creature that drew it, must make a DC 15 Constitution saving throw. Each creature takes 4d8 necrotic damage on a failure, or half as much on a success. Once Bleed has dealt damage this way, this effect doesn't trigger again until the next dusk.

At the start of a creature's turn, if that creature is holding this sword and it is not in its sheath, the creature takes 2d8 necrotic damage.

Cursed item. Sheathing or purposefully dropping this sword requires a successful DC 15 Wisdom check. On a failure, the creature wielding Bleed cannot attempt to drop it or sheathe it again until its next turn. If the creature attempting to drop or sheathe Bleed isn't attuned to it, the DC is 20 instead. The DC of this check is reduced by 5 for every 100 damage it deals after being drawn, to a minimum of 5.
