Thanatotic Titans
Thanatotic titans were created as weapons for the divines to use against mortals. Whereas other titans such as the kraken or tarrasque were used to unleash divine wrath or to wage war against other deities, thanatotic titans were used in a much more reserved manner.
Instead, their primary purpose was to serve as a means for balancing the cosmic flow of life and death. Once awakened or summoned, the thanatotic titan only has one goal, to consume the souls of all living creatures in their path. Only by divine command will these towering juggernaut ceasing their devastating campaigns.
Soul Harvesters
Unlike many entities from the Lower Planes, thanatotic titans do not consume or corrupt the souls they acquire. Instead, they could be viewed as living repositories for souls, which they bring back to their divine masters.
Agents of Extinction
Because of their abilities, many thanatotic titans are often unleashed onto the lower planes by many deities to reclaim souls bound to the Nine Hells or Shadowfell. Other divines, especially with those who oversee the domains of life and death, use these titans to safeguard the souls of their respective domains. In some more extreme cases, thanatotic titans were unleashed upon the planes in conflicts where the divines sought to scourge creation itself in an attempt to remake the world to their likeness.
Thanatotic Titan Tactics
Battling a thanatotic titan is akin to fighting a force of nature rather than an ordinary creature. There is little one can do to cause a thanatotic titan to deviate from its path. Once unleashed, the thanatotic titan's primary goal is to target large population centers wherein it can harvest more souls. In most circumstances, this will be its primary goal unless given a specific directive by its master.
Keen Mind
However, despite its somewhat simplistic goals, the thanatotic titan has an uncanny intelligence which prevents it from being deterred by simple distractions. Thanks to its colossal size and strength, the thanatotic titan will largely ignore minor threats, but it is intelligent enough to underestimate its opponents.
Battle Strategies
When dealing with numerous opponents, the thanatotic titan's best defense is its Soul Harvest ability, which is capable of annihilating scores of enemies in a matter of seconds. If its enemies are beyond the range of this attack, it will resort to using its thanatotic slams and bursts to quickly eliminate its foes. It prefers to use its thanatotic bursts to eliminate small groups at a time.
For more dangerous opponents, it will resort to use its ghastly limbs to grasp its foes and consume it in its colossal maw located in its stomach.

Legendary Resistance(3/day) If the thanatotic titan fails a saving throw, it can choose to succeed. Upon using this feature, it can make a Thanatotic slam as part of that action.
Magical Attacks The thanatotic titan's attacks count as magical for the purpose of overcoming resistance
Magic Resistance The thanatotic titan has advantage on saving throws against spells and magical effects.
Siege Monster The thanatotic titan can deal double damage to buildings and objects
Soul Sight The thanatotic titan can pinpoint the location of any creature within 120ft. of it.
Soul Siphon When a creature within 30ft. of the thanatotic titan is reduced to 0 hitpoints, it is instantly killed. Creatures killed in this manner cannot be revived, resurrected, or turned into an Undead by any means short of a Wish spell. If the thanatotic titan is destroyed, the souls of the slain are no longer bound.
Titanic Nature The thanatotic titan doesn’t require air, food, drink, or sleep
Titanic Strength The thanatotic titan has advantage on Strength ability checks and saving throws against creatures at least one size or smaller than it.
Actions
Multiattack. The thanatotic titan can make two thanatotic slams. Alternatively, it can make a two thanatotic bursts instead
Thanatotic Slam. Melee Weapon Attack: +17 to hit, 10ft. range, one target. Hit: 25(5d6+8) bludgeoning damage + 23(5d8) necrotic damage. On an hit, Huge and smaller creatures are grappled and restrained(escape DC 25). The thanatotic titan can only grapple and restrain two creatures at a time in this manner.
Thanatotic Burst Creatures in a 10ft. radius around a spot the thanatotic titan can see within 300ft. must succeed a DC 22 Charisma saving throw. On a failed saving throw, it takes 41 (9d8) necrotic damage and half as much on a success.
Soul Harvest(Recharge 5-6) Creatures within a 60ft. radius around the thanatotic titan must succeed a DC 22 Charisma saving throw. On a failed saving throw, the target drops to 0 hitpoints. On a successful saving throw, a creature takes 41 (9d8) necrotic damage.
Bonus Action
Death's Maw Melee Weapon Attack: +15 to hit, 5ft. range, one grappled target. Hit: 58(9d10+8) piercing damage. On an hit, Huge and smaller creatures are swallowed and the grapple ends.
While swallowed, the creature is blinded and restrained, it has total cover against attacks and other effects outside the thanatotic titan, and it takes 32 (7d8) necrotic damage at the start of the thanatotic titan's turns. If this reduces a creature to 0 hitpoints, it is instantly killed and its physical body is destroyed.
At the end of its turn, when the thanatotic titan takes damage from a swallowed creature, it must succeed a Constitution saving throw equal to half the damage dealt(minimum of 10). On a failed saving throw, the thanatotic titan regurgitates all swallowed creatures, which fall prone in a space within 10 feet of the thanatotic titan. If the thanatotic titan dies, a swallowed creature is no longer restrained by it and can escape from the corpse using 15 feet of movement, exiting prone.
Legendary Actions
The thanatotic titan can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The thanatotic titan regains spent legendary actions at the start of its turn.
Titanic Stride(1 action) The thanatotic titan can move up to half its movement speed.
Thanatotic Slam(2 actions) The thanatotic titan can make a thanatotic slam
Devour(2 actions) The thanatotic titan can make a Death's Maw attack against a grappled creature
Divine Resilience(3 actions) The thanatotic titan can end one spell effect or status condition on itself.
