Ancestral Dragon Talisman

This relic was forged by the ancient dragon cults of old, who sought to obtain the power of dragons for themselves. When attuned to the talisman, you can transform into a dragon based on your ancestry, as if you were the target of the True Polymorph spell. This form does not require concentration to maintain, and can last for up to 10 minutes. Using the talisman to assume the form of an ancestral dragon can be done once per long rest. The age of the dragon is determined as follows:

As a wyrmling, your claws are still underdeveloped. You cannot wield an item such as a wand or a weapon, but you can still wear (and attune to) items such as necklaces, rings, and ioun stones.
As a young dragon, your claws have become more developed, and you are capable of wielding items that your regular form can wield, such as a wand or a weapon.
Due to a curse placed on this talisman by Klauth, to spite those who would contest his power, the artifact has the negative effect of turning you into a baby for 1d6 days once you revert to your normal form.
As a baby, your mental stats are unchanged. However, your Strength and Dextrity scores become 4, and your Constitution becomes 10. Your movement speed is replaced with a crawling speed of 5. You cannot transform into an ancestral dragon while you are a baby.
The curse on this item can only be broken if it is imbued in the fiery breath of an ancient red dragon or through the use of the Wish spell.
