Urthugool
Cultists so often flirt with the Far Realms that it is a rite of passage to become one with them. Urthugool are cultists who've succumbed to the far realms, remorselessly trading their sanity for the power they have so long worshipped. Such a joyful event is becoming an Urthugool that The Chosen cry out in joy even as their bones are liquified and tentacles begin to pierce through their skin.
Captors Urthugool grapple their opponents and drag them into their dens, rather than killing them outright. Once their opponent has successfully been acquired and prepared for initiation, Urthugool reveal to them what lies below the hood - an event that no one has returned to describe.

Multiattack. The urthugool makes two tentacle attacks. It can replace any of these attacks with Fling.
Tentacle. Melee Weapon Attack: +9 to hit, reach 15 ft., one target. Hit: 13 (2d6 + 6) bludgeoning damage, and the target is grappled (escape DC 17). Until this grapple ends, the target is restrained. The urthugool has 6 tentacles, each of which can grapple one target.
Fling. One Large or smaller object held or creature grappled by the urthugool is thrown up to 30 feet in a direction of the urthugool's choice and knocked prone. If a thrown target strikes a solid surface, the target takes 3 (1d6) bludgeoning damage for every 10 feet it was thrown. If the target is thrown at another creature, that creature must succeed on a DC 17 Dexterity saving throw or take the same damage and be knocked prone.
No Escape (Recharge 5-6). The urthugool makes up to five attacks with its tentacle, provided that each attack targets a different creature.
